Every patient begins with the initial evaluation; the two memberships are ongoing monthly concierge care. Additional follow-ups are $350 (30 min) or $650 (60 min). We provide a detailed superbill you can submit for possible out-of-network reimbursement, and a Good Faith Estimate on request.
We're in the credentialing process with Horizon BCBS NJ, Aetna NJ, and Cigna / Evernorth NJ.
Until each panel finalizes, New Jersey patients receive an out-of-network superbill to submit for partial reimbursement under their out-of-network benefits. This is the bridge that lets you start care now rather than waiting for credentialing to complete.

What's a superbill? It's an itemized receipt for your visit with the codes your insurance company needs. You pay Sigma directly, then submit the superbill to your insurer, who may reimburse part of the cost under your out-of-network benefits. We prepare it for you.

You can arrange and prepay an evaluation for a family member, a partner, or a college student. They complete their own private intake, and the care stays between them and their clinician.
